Telehealth Bandwidth Requirements: What Clinics Need to Plan For

Telehealth has moved from an emergency workaround to a permanent part of how many clinics deliver care — but the network behind it doesn’t get the same planning attention as the clinical workflow does. A choppy or dropped telehealth call isn’t just an inconvenience; it can affect the quality of a visit and the patient’s confidence in the practice. Here’s what actually matters when planning network capacity for telehealth.

Why Telehealth Has Different Requirements Than General Office Use

Video calls are far more sensitive to network conditions than most other traffic a clinic generates. Email and file access tolerate brief slowdowns without anyone noticing. Video and audio don’t — a momentary network hiccup shows up immediately as a frozen frame or garbled audio, exactly when a provider is trying to communicate something important to a patient.

This means a network that feels “fine” for general office use can still produce a poor telehealth experience if it wasn’t specifically planned with video traffic in mind.

What Actually Drives Telehealth Call Quality

Available bandwidth. Each concurrent video visit needs a reasonably predictable amount of bandwidth, and this needs to be available on top of everything else happening on the network at the same time — EHR access, other staff browsing, any other video calls happening simultaneously.

Network stability (jitter and packet loss). Raw bandwidth isn’t the whole story. A connection can have plenty of available bandwidth and still produce poor call quality if it’s unstable — inconsistent timing (jitter) and lost data (packet loss) are usually the real cause of garbled audio and frozen video, more often than insufficient bandwidth alone.

Quality of Service (QoS) configuration. Without QoS prioritization, a large file transfer or software update happening elsewhere on the network can compete with a telehealth call for bandwidth at exactly the wrong moment. Configuring the network to prioritize video/voice traffic prevents this.

Wi-Fi reliability, if the visit happens over wireless. If providers conduct telehealth visits from a laptop or tablet over Wi-Fi rather than a wired connection, Wi-Fi coverage and interference become part of the equation — a weak signal in a specific exam room can produce call quality issues that have nothing to do with the internet connection itself.

Planning for Concurrent Visits

The most common planning mistake is sizing network capacity for a single telehealth visit instead of the realistic number of concurrent visits a busy clinic might run at once. If your clinic has multiple providers who might each be conducting a telehealth visit simultaneously during a busy block, your network needs to comfortably support that peak load — not just the average.

A useful planning exercise: identify your busiest realistic hour (multiple providers, multiple concurrent video visits, plus normal EHR and administrative traffic) and make sure your network design is sized for that peak, not the average day.

Compliance and Encryption Considerations

Telehealth platforms generally handle encryption of the video/audio stream itself, but the network needs to reliably support whatever connection method the platform requires without introducing bottlenecks that degrade the secure stream. This is less about the network “doing” the compliance work and more about making sure the network doesn’t become the weak link that causes a provider to work around proper procedure out of frustration with call quality.

  3. Are exam rooms or telehealth stations using a wired connection, or relying on Wi-Fi — and if Wi-Fi, has coverage actually been verified in those specific rooms?
  4. Has anyone tested call quality during a realistic busy period, rather than just during a quiet test call?
  5. Is there a clear escalation path if a provider experiences recurring call quality issues — is it treated as a known network issue to investigate, or dismissed as a one-off?

Getting Ahead of It

Telehealth network issues tend to surface gradually — a complaint here, a dropped call there — until they become a recurring frustration for providers and patients alike. A network assessment focused specifically on video traffic, concurrent load, and Wi-Fi coverage in clinical areas catches these issues before they affect care.
